Patient Forms

At Health Xpress, we streamline your check-in process with convenient electronic forms. To ensure your information is secure and your appointment runs smoothly, we handle all necessary paperwork digitally before your visit.

When you schedule an appointment, we will send you the required forms through our secure Electronic Medical Record (EMR) system. These forms include:

Consent To Treat Form
Patient Demographics

You can complete and sign these documents electronically from the comfort of your home. As part of this process, you will also be required to upload a copy of a valid ID. Completing these steps in advance saves you time and allows us to focus entirely on your health needs during your visit.
